Refer a Patient or Client for Meal Support

If you coordinate care and need someone to actually answer the phone about food, start here. This form opens a referral conversation about coverage of an address, dietary fit, timing, and cost — it is contact coordination only. It is not a clinical intake, and it must not be used to send protected health information, diagnoses, or medical records until a compliant workflow is established and agreed in writing.

Where we help most

Start a referral conversation

Tell us the organization, your role, service area, estimated referrals per month, and program type. A real person from the kitchen replies. If we cannot serve the address or the diet, we will say so plainly. Do not include patient identifiers, diagnoses, or records.

Please note: SmashMeals is a direct-pay prepared-meal company. We are not in-network with, contracted by, or endorsed by any health plan, Medicare Advantage plan, Medicaid program, or managed-care organization, and we do not bill insurance or verify eligibility. Meal benefits, where they exist, depend on the individual plan, program, county, plan year, and authorization. Information on these pages is general education, not insurance, medical, or legal advice.

Frequently asked questions

Can I send patient records or PHI through this form?
No. This web form is for contact and referral coordination only. We do not accept protected health information, diagnoses, or medical records until a compliant workflow is established and agreed in writing.
Does SmashMeals accept plan authorizations or bill insurance?
Not today. We are direct-pay and not in-network with any plan or program, so referrals are handled as direct-pay arrangements unless and until a contract exists.
Which areas can you serve?
Local delivery and pickup across the Tri-Cities region of Northeast Tennessee, shipping statewide in Tennessee, and FedEx Ground shipping beyond it. Ask us about a specific address first.
Can you handle strict gluten-free needs?
Our entire kitchen is gluten-free — we do not bring wheat, barley, or rye into the building. A dedicated gluten-free facility is not an allergen-free facility, so check each meal's label for other allergens.
