TennCare CHOICES and Home-Delivered Meals

TennCare CHOICES is Tennessee's Medicaid long-term services and supports program for older adults and adults with physical disabilities. Home-delivered meals are among the home and community based services that a CHOICES care plan can authorize for members who meet the program's eligibility and level-of-care criteria. Meals are arranged through the member's care plan: the managed-care organization and care coordinator — BlueCare, UnitedHealthcare Community Plan, or Wellpoint — determine authorization and the available network provider options under the member's plan and program rules. How a CHOICES meal authorization typically happens

  1. The member is enrolled in CHOICES through TennCare and assigned to a managed-care organization.
  2. A care coordinator completes an assessment and builds a person-centered care plan.
  3. If nutrition need is documented in that plan, home-delivered meals can be authorized as a service.
  4. The MCO determines authorization, the available network provider options, the number of meals, and the review date under the member's plan and program rules.

Because the authorization runs through the care plan, the fastest route to an answer is the care coordinator — not a customer service line and not the vendor. Eligibility, service limits, and vendor lists are set by TennCare and the MCO, and they change.

The celiac and dedicated-gluten-free gap

Some covered vendors may use shared facilities, or may not offer a dedicated gluten-free production environment. A "gluten-free" description can refer to the recipe rather than to the kitchen it was cooked in. If cross-contact is medically important, verify the vendor's actual kitchen controls directly with the plan or the vendor before meals start.

A dedicated gluten-free facility is not an allergen-free facility — check each meal's label for dairy, soy, egg, nuts, and other allergens. We are a direct-pay option some families use when a covered vendor cannot meet that need, when a short-term benefit has run out, or when they are feeding a parent in another state.

Questions for a CHOICES care coordinator

Call the member services number printed on the back of the insurance card and ask these questions in order. Write down the answers and the reference number for the call.

  1. Does this plan include any home-delivered meal benefit this plan year — post-discharge, chronic-condition, or a grocery/food allowance?
  2. What triggers it: an inpatient stay, a documented diagnosis, a care-manager referral, or enrollment in a specific program?
  3. How many meals, over how many days, and does the clock start at discharge?
  4. Which vendors are approved, and can any of them prepare meals in a dedicated gluten-free facility?
  5. If no approved vendor can meet a celiac-level dietary need, is there an exception, out-of-network, or reimbursement pathway?
  6. What documentation does the plan need, and who submits it — the member, the discharge planner, or the care manager?

If the plan cannot supply a vendor that meets the dietary need, ask for the answer in writing. That letter is what a care manager or an appeal typically needs.

If meals aren't authorized, or the vendor can't go gluten-free

Families in East Tennessee may choose to purchase meals privately. Smash Meals is based in Kingsport: pickup is free, local Tri-Cities home delivery is $15, and FedEx Ground reaches 30 states + DC for a $30 flat rate. Order by Thursday at 11:59 PM Eastern for the following Tuesday. There is no subscription and no minimum order, so a family can order a single week during a rough stretch.

Ask the care coordinator to note that in the care plan and to confirm in writing that no contracted vendor can meet the need, then ask what exception process exists. That documentation is what any appeal or alternative arrangement is built on.
